We support patients and their families that have been diagnosed with Cancer and, are either undergoing treatment or in remission.
This can be done on a 1:1 basis or in the support groups that we run.
What can a Cancer Care Co-Ordinator help with?
- Chasing hospital appointments and follow ups.
- Signposting to other departments for specific queries
- Financial advice eg benefit information etc
- Supporting patients to take control of their health.
- Getting patients to focus on their own priorities and the things that affect their wellbeing.
- Loneliness and isolation caused by their cancer diagnosis.
How can patients access our services?
If you need support from our team please contact your GP Surgery and they will either give you our contact details or contact us to give you a call.
We also run regular support groups. Details of which you will find at the surgeries or on this PCN website.
