A new Physical Health Team pilot has launched in mid Essex to help improve the health and wellbeing of people with serious mental illness.
A team of healthcare assistants have been recruited to work with our primary care network to deliver physical health checks. These will take place in GP surgeries, or they can visit you in your home if needed. They will follow up with telephone calls to make sure you have the support you need.
During the physical health check, the team can look at a range of things including:
- BMI
- smoking and alcohol use
- nutrition and exercise
- blood pressure
- sexual health
- medication review
- vaccinations programmes
- public health screening programmes
The Physical Health Team will work with you on positive health outcomes. This means they can give you advice and support to help manage or improve your health and wellbeing needs.
The Physical Health Team is overseen by a Registered Mental Health Nurse. People with serious mental illness who are eligible for this service will be contacted by the Physical Health Team.
